PREOPERATIVE DIAGNOSIS: Nasal obstruction secondary to deviated septum & hypertrophic turbinates.
Bilateral compensatory hypertrophic turbinates.
External nasal deformity.
Septal Deviation.

PROCEDURE:

  1. Open rhinoplasty with bilateral concha cymba composite grafts.
  2. Complex septal reconstruction, submucous resection with major partial vomerectomy for correction of deviated nasal septum.
  3. Submucous resection and partial resection of right inferior turbinate.
  4. Submucous resection and partial resection of left inferior turbinate.

FINDINGS: Poor tip projection, hanging, ptotic tip, retracted ala R>L, hypotrophic, thin abnormal right lower lateral cartilage (LLC), ptotic tip, septal deviation to left, and thick skin.

COMMENT: This patient wished to improve her breathing problems and improve the appearance of her nose. Post-operative photos reveal a softer profile. Patient has reported improvements in breathing as well.
