1. Brow ptosis & asymmetry.
2. Nasal obstruction secondary to external/internal valve collapse, residual deviated septum and hypertrophic turbinates.

Procedure:

1. Browlift with endotyne fixation, platelet gel application.
2. Revision Septal reconstruction for correction of deviated nasal septum and harvest of septal graft.
3. Repair of nasal vestibular stenosis with reconstruction of external valve collapse with placement of bilaterallateral crural strut grafts.
4. Repair of nasal vestibular stenosis with reconstruction of tip support with placement of columellar strut.
5. Repair of nasal vestibular stenosis with reconstruction of internal valve collapse with placement of bilateralspreader grafts.
6. Graft, right earcartilage.
7. Right and left deep temporalis fascia harvest (via brow lift).
8. Repair of nasal vestibular stenosis with reconstruction of right external valve collapsewith placement of right alar rim graft.
9. Submucous resection and partial resection of right inferior turbinate.
10. Submucous resection and partial resection of left inferior turbinate.
11. Repair of nasal vestibular stenosis with reconstruction of right internal valve collapse with placement of right middlevault and pyriform aperture cartilage onlay grafts.
12. Secondary Rhinoplasty, platelet gel application.

Findings: Pre-op for eyes: right brow higher than left. Left caudal deviated septum with absentbony septum, Bilateral lower lateral crura approx. 6 mm in width with weak right lower lateral cartilage greater than left lower lateral cartilage (LLC) and significant depression right lateral tip and right middle vault, narrow middlevault (right greater than left), residual bony and cartilaginious hump atrhinion.

Comment: This patient wished to give her eyes a fresher appearance. She also desired to correct a deviated septum and nasal hump. Post-operative photos reveal a straighter nose with a nicer, softer look. In addition, eyes are improved and compliment the face nicely.
