Nicola Stratton: When I look in the mirror, I always think to myself I need some kind of help _____. I do feel quite low with my body size and also in the face, as well, definitely. I'm getting to the age now where I think nothing can help apart from the knife.

Announcer: The first stop is Spalding Drive , L.A. 's equivalent to Harley Street . This is where you find the surgeons to the stars.

Paul Nassif: Nicola.

Nicola Stratton: Hi.

Paul Nassif: Dr. Nassif.

Nicola Stratton: Oh, nice to meet you.

Paul Nassif: Welcome.

Announcer: More accustomed to celebrity clientele, Dr. Nassif has drastic plans to radically transform Nicola's face. He's going to sculpt and refine her features.

Paul Nassif: I like your profile. You have a lot of skin here.

Nicola Stratton: Yep.

Paul Nassif: If we were just going to go ahead and try to remove a lot of that skin by doing an upper eye blepharoplasty, your eyebrow will drop. So I'm going to stabilize and bring the eyebrow up with a brow lift. At the same time, I'll weaken the muscles here.

All right. So I'm gonna be kind of touching your nose here. So the skin of your nose is a little bit thick. The good part about that is it camouflages sometimes irregularities in the cartilage. It will come down a little bit, though.

Nicola is going to undergo an endoscopic brow lift, an upper eyelid lift. She's going to have her ears pinned back. She's gonna have a rhinoplasty. She's also gonna have liposuction to her neck and her jowl.

Announcer: The most dramatic difference to her face will be her nose. Dr. Nassif uses state-of-the-art technology to show Nicola what the alterations will look like.

Announcer: And that's only the first consultation. There's much more to come. Over the next two days, Nicola will consult with all her doctors. Next up, eminent plastic surgeon Dr. Gerald Minniti. Can he fulfill Nicola's desire to have bigger breasts?

Gerald Minniti: Wow, that's good. Great. You have very nice, shapely breasts. I think they're in good position. The nipples, they have pretty good symmetry. The only thing I noticed is that this breast is a bit smaller than this one. And I think that can be compensated adequately with an implant that is slightly larger on this side than this side in order to achieve better symmetry.

But other than that, the shape and appearance of your breasts is very satisfying. And I think that implants alone will give you a nice result in terms of augmentation.

Nicola's very tall. She's over six feet tall and she's an airline stewardess. And she wants to look pretty. She wants to look sexy. So I want to make her proportionate. And because of her size, I think she needs a relatively large implant. This implant here is 300 and this is 375. I think for Nicola, she's gonna need an implant that's something like this in volume.

Nicola Stratton: Would you think I'm about to go into a C or a D cup? I mean, personally, I like the D, but I'm thinking more of, you know, because of my body shape.

Gerald Minniti: Right, right. Well, you know what I'm gonna have you do is pick out some photographs and show me what you find aesthetically pleasing.

Nicola Stratton: Okey-doke.

Announcer: In L.A. , picking your new breasts is like catalog shopping. Breasts come in every shape and size imaginable. Nicola has masses to choose from. Bigger boobs are the fulfillment of a lifetime's desire. Knowing what she's going to get is the perfect end to a hectic day.

Announcer: Nicola's barely stopped since she got to L.A. just three days ago. Today, she faces another round of consultations. But she's a little apprehensive about her first appointment.

Nicola Stratton: The thing that I don't really like about dentists is the noise, for one, and the feel that you're at their complete mercy. They're armed.

Announcer: But things are a little different when you're granted an audience with the dentist to the stars.

Bill Dorfman: How's it going?

Nicola Stratton: Oh, okay. I'm a bit nervous, that's all.

Bill Dorfman: Why?

Nicola Stratton: Oh, just a childhood thing with dentists, really. A bit nervous, but I trust you wholeheartedly.

Bill Dorfman: Really?

Nicola Stratton: Yeah. I do trust you.

Bill Dorfman: Go ahead and grab a hold of this and I'm going to put this in so I can see, too. Okay? It's called a Seymour retractor. Cause when you put it in, you see more. So when I look at your teeth, the first thing I notice is the color. They've got to be whiter. They actually say that your teeth should be about the same white and the whites of your eyes. If that's the case, we've got a long way to go. So let's bleach them. And I think if we do a little bit of reshaping down in here, and maybe a few porcelain veneers on the upper there, they could look pretty good.

Nicola Stratton: Will it hurt?

Bill Dorfman: Absolutely not.

When you look at Nicola's teeth, it looks like she's stressed out. I mean, she's got all this wear and tear on her teeth. We'll whiten her teeth. I'll do eight porcelain veneers. She'll go from ordinary to extraordinary. She'll look like a super model.

Announcer: Before there's any chance of that happening, Nicola needs help from another doctor.

David Amron: And let's see if you're a good candidate for liposuction.

David Amron: Because you're so lean and in shape in other areas, and you really are, but you're tending to store more fat in these areas over here. So this is really what liposuction ideally is all about.

Announcer: The consultations are over. Everything's happening so fast, that to complete her drastic transformation in such a short period of time, Nicola will be under the knife tomorrow.

Nicola Stratton: Tomorrow's gonna be the day that changes for the rest of my life, really. I can't wait to get there. I know there's no saying you can't wait to get into surgery. Most people don't like to do it. But there's something coming out at the end of it and that is the bit that I'm really, really looking forward to. There'll be a brand new me.

Announcer: The surgeons plan to transform Nicola in just 20 days. Remarkably, her face and breast surgery will be carried out simultaneously. Then she'll have extensive liposuction, and finally, teeth bleaching and veneers. It's 5:00 in the morning, and Nicola has an intense day of surgery ahead of her.

Paul Nassif: I'm gonna actually now come in and mark your face. All right. And then Dr. Minniti is gonna follow me and he's gonna mark your breasts. And then we're gonna rock and roll.

Announcer: Nicola will have her face and breasts done in one surgery with both doctors operating at the same time. Dr. Nassif's markings are his blueprint for the operation.

Paul Nassif: Nicola's I'm sorry about that. So right now what we're doing is we're marking the little nerves that control sensation to your forehead. And I'm just marking there cause that's one little area that I have to protect and watch out for.

Announcer: During the operation, Dr. Nassif also plans to inject Restylane into Nicola's lips.

Paul Nassif: I'm looking now. I can see where I'm off a little bit. And I'm gonna bring this one up just a little bit like that. All right, young lady. We're done. _____ I'm gonna go find Dr. Minniti. I'm gonna get him to come in here to do his handiwork, now.

Gerald Minniti: Okay, let's see here. Well, first I'm gonna draw a line right down the middle here.

Nicola's gonna get saline filled implants, which are a device that has a outer rubber shell that's empty, and I fill it with salt water solution. And the implant is put in through the belly button and inflated.

That's perfect. You are all set.

Nicola Stratton: Thank you.

Announcer: Nicola is ready for the op.

Nicola Stratton: I'm starting to feel a bit nervous, now. Only a little bit, though.

Paul Nassif: Well, our game plan with Nicola today, I'm going to start off with her liposuction of the neck and the jowls first so that I can finish that lower part of the face so that Dr. Minniti can come in and do his breast work.

Announcer: It's quite extraordinary to have two of the world's leading cosmetic surgeons simultaneously carrying out one drastic transformation. It's going to be a long day.

Paul Nassif: Give me a 15 blade. So we're going to start making a small little incision right in the ear area. I'm making little tracks for the liposuction. Okay, here we go. What I'm doing is making sure we have symmetric areas of liposuction.

Announcer: Dr. Minniti scrubs in for his part of the operation.

Gerald Minniti: Dr. Nassif and I get along very well. We're both jovial people and we enjoy operating. It's fun to operate. And even though it's a close environment, we get along very well. We seem to work together well. It's like dancing. You have to do a little bit, stop. I do a little bit, stop. Sometimes we can work together. But we enjoy it.

Paul Nassif: But I'm a better dancer than you.

Gerald Minniti: That's right.

Announcer: With both doctors operating at once, this will reduce the time Nicola spends under the anesthetic and therefore, speed her recovery. Dr. Nassif begins work on Nicola's brow lift, while Dr. Minniti preps for her new breasts.

Gerald Minniti: I have these relatively long scissors. And what I'm doing is I'm just lifting the fatty tissues right off the muscle, right up her belly, right over her ribs, right up to the bottom of her breasts. And I lift the breast up, which also lifts that underlying muscle that I'm gonna eventually get underneath. Now I'm gonna backtrack and sort of spread these big, huge scissors which are gonna make that tunnel that I just created nice and spacious.

Paul Nassif: This is the endoscope right here. This is a camera telescope. And I'm just elevating tissue.

Announcer: The endoscope allows Dr. Nassif to operate with precision.

Paul Nassif: There we go.

Announcer: He snips the muscle to weaken the frown lines that cause the deep groove in Nicola's forehead.

Paul Nassif: I'm being very careful how I do this because if we're too aggressive, we could actually cause a little depression in the area.

Announcer: Dr. Minniti prepared the implant shell for insertion. Inserting the implant empty allows Dr. Minniti to go through the belly button, leaving no visible scars. Once the implant is in position, he'll fill it with saline.

Gerald Minniti: So I went right up the tunnel. It's now under the pectoralis major muscle, and we're going to start the process of filling it. This implant requires at least 510. Sounds big, but she's a tall girl and she's looking for a small D cup.

Announcer: Dr. Minniti is one of only nine surgeons in the world who have perfected implants through the belly button.

Gerald Minniti: We'll go to 510 and stop. Cap. I'm just gonna loosen these implants in the pockets. I think I need to make some small adjustments to them. I'm not so concerned about where the tops of the implants are on her chest, other than the fact that they look equal and symmetrical. But I have to be mindful of where the bottoms are. Because eventually, the bottoms are going to be containing all the water of the implant. Filling tubes are removed.

Announcer: Nicola's breast operation is almost complete.

Gerald Minniti: It's time to finish the operation and close the hole that we created, the one and only hole, here in the belly button.

Announcer: Remarkably, there are no visible scars.

Gerald Minniti: I am going to leave you now and we're gonna

Paul Nassif: Well, I will see you soon.

Gerald Minniti: Of course.

Paul Nassif: I've just marked now the crescent of skin that I'm going to be removing on the upper eyelid. Perfect. And here we go. So I made the incision through the skin. Now we're going to remove that skin using what we call a Colorado tip needle. So we're removing skin and trying not to remove much muscle at all. Okay, here's our skin. All right, fantastic. This is a very fine stitch which will be removed in seven days, cause we want to make sure we give her a perfect little thin line there. And I take my time and I do a beautiful job closing this.

Announcer: The next phase in the operation is to pin Nicola's ears back.

Paul Nassif: We have to weaken the cartilage here. So we're gonna do that by, shall we say, abrading the cartilage. This will help the cartilage bend towards the direction we want. What the stitches are gonna do are gonna help mold the cartilage for us. We tighten the suture, it's gonna bend the ear just like that.

Okay. Right now, we're gonna start the rhinoplasty part of the procedure. For the first part of making the cartilage of the tip smaller is remove this soft tissue layer which is rather thick. And just by doing that, that will make the nose thinner itself. Now we're going to measure.

And the most important thing is really not what we remove, but what will we leave behind. Because if you take too much cartilage out, the problem with that is maybe not right now but in a year or two, you'll start seeing deformities of the tip of the nose. Okay, perfectly symmetric. The tip is already smaller.

Announcer: To finesse the shape of her nose and give it more definition, Dr. Nassif chisels the bone.

Paul Nassif: Go. Go. Stop. So the mobilized bone heals to its new location which is closer together. We're taking a bone that's wide and bringing it closer. And now we have to set it by placing a cast over it. Nic, we're all set, here.

The procedure went fantastic. In the beginning, we did a combined surgery with Dr. Minniti. And of course I did all the face work and it went great.

Nicola Stratton: Oh. My chest feels really heavy, but good.

Paul Nassif: It's taken a little over seven hours to complete her procedures.

Nicola Stratton: It's unbelievable. Absolutely unbelievable. No pain in my face. It just feels like I've got a crash helmet on. That's it, a bit of pressure. No pain whatsoever.

Announcer: Coming up, Nicola gets the body of a super model.

David Amron: That's about a liter and a half of fat.

Announcer: And she starts to see an incredible transformation.

Nicola Stratton: It's all I can say. Oh! Oh, it's amazing.

I also have to use this to clean out my nose, because obviously I can't blow it in case it falls off. So this is my little sucker upper that I have to use if my nose starts to run.

Announcer: It's the midway stage in Nicola's L.A. transformation. And there's no let-up in the schedule to make her beautiful. It's Nicola's dream to be sexy and glamorous. And to help her achieve that goal, Dr. David Amron has big plans. He's going to get rid of her excess fat and flab. And she can finally say goodbye to those bingo wings.

David Amron: But to give her amazing results, I've got to really reduce her all the way around three dimensionally, and kind of pinch in that whole area. And this is what's gonna create a whole new shape for Nicola and really give her a killer body.

Nicola Stratton: I'm not expecting immediate results. I know it won't be cause I'm still gonna be swollen. But I know he's gonna take a lot of fat out today. But I forgot to say how much fat he does have to take out.

David Amron: I don't expect to get a lot of fat out, probably about a liter, which is about two and a half pounds of pure fat. But it's gonna do an amazing job in terms of giving her a better shape.

All right, Nicola. We're gonna start putting local anesthesia in.

Announcer: Dr. Amron has pioneered liposuction under local anesthesia. This means Nicola will be awake for the entire operation.

David Amron: How is it, not too bad?

Nicola Stratton: It just feels a little bit weird, really, like somebody pressing on my stomach.

Announcer: It'll be over in less than two hours.

Nicola Stratton: It's not painful. It feels like my stomach's going numb, very numb.

David Amron: Nicola, how are you doing?

Nicola Stratton: Okay.

David Amron: Okay. We're getting some beautiful fat out.

Nicola Stratton: Oh, _____. Do you have to go all around my stomach doing that?

David Amron: Yep.

Nicola Stratton: Uhh. [Laughs]

 

David Amron: Liposuction is probably not the most accurate term. It really is sculpting. I think there's a lot of art to doing great liposuction, a lot of finesse. But it does take some elbow grease to pass this instrument underneath and remove the fat. The fat is embedded in a fairly dense connective tissue matrix, which the local anesthesia helps to loosen up, but also I'm kind of busting through it with my cannula with each pass.

Announcer: Dr. Amron's technique of doing liposuction under local anesthetic is so revolutionary that it's been coined lunchtime lipo.

David Amron: I feel it's a much safer way to do it. It's a much cleaner, more efficient way to do it. The recovery is definitely faster. She's gonna walk out feeling good. And then from results standpoint, it allows me the best results.

Nicola Stratton: It feels weird. I feel very shaken .

David Amron: I'm very careful as I'm sculpting to really respect the skin a lot. You should never see me poking the skin from underneath. And I feel that really this is what allows me to get such a smooth contour. Now even though my right hand is the hand that's going back and forth, my left hand is really securing the skin and always feeling where the tip of my cannula is, where the suction is going on, which is right there, this little tiny hole on both sides of this instrument. You can see the nice orangey fat coming out. Yeah, this looks great. Really nice. It's giving a nice pinch into her waist. Okay.

Announcer: Nicola is shaping up incredibly well. Now, it's time for those bingo wings to go.

David Amron: Lie straight. Lie straight. I just want to check this arm here. Great. We're done.

Nicola Stratton: Yay.

David Amron: Look.

Nicola Stratton: Oh, my Lord! How much is that altogether, do you think?

David Amron: It's about a liter and a half of fat.

Nicola Stratton: A liter and a half?

David Amron: Yeah.

David Amron: I think Nicola's liposuction went great. She was in positions like this with her arm above her head. So it really allows me to bring this whole area down three dimensionally. And the arms, where she was really out of proportion, are gonna greatly improve and retract up. So she did great.

Announcer: Doctors orders have Nicola in bed to recuperate for the rest of the day.

Nicola Stratton: Dr. Amron showed me what he had sucked out of my stomach and it was horrific. Talking about it now is making me feel sick. It was awful. And that's through eating pure junk. I could visualize putting that fat in that looking like that back in my stomach, and I don't want it back in. It was awful, absolutely awful. What horrific _____.
