Spalding Cosmetic Surgery and Dermatology - Our Doctors

Female Speaker: This is Susan's story. Forty-five year old Susan Fieldhouse from Halifax is one of the thousands of women who applied to Brand New You. She feels self-conscious and embarrassed by her looks.

Susan Fieldhouse: I feel dumpy, frumpy, not really got that much shape, I don't think. Boobs maybe stick out just as much as me tummy.

Female Speaker: Susan despairs her aging looks make her appear more masculine than feminine and desperately wants plastic surgery to transform her. Deep set wrinkles and heavy eyes make her look more like 65 than 45. Her large, crooked nose and prominent moles make her feel unattractive and to top it off, she's blind as a bat.

Worse still, gravity has reduced her breasts to droopy sacks. Plus four kids and cesarean have wreaked havoc with her waistline and left her belly sagging. To create the brand new Susan Fieldhouse will cost a phenomenal $300,000.00. Work ing mom Susan has been self-conscious about her looks all her life and has always hated her prominent features.

Susan Fieldhouse: I mean right from the very beginning, I've always wanted to change my nose shape without a doubt. Everything else has just kind of gradually built up over time. Sometimes I feel a bit self-conscious. If you are walking past anyone and I catch myself in the shop window or something, I see someone who is dumpy, frumpy. I think it's just probably lack of self-esteem.

Female Speaker: Susan's confidence took a battering during her first marriage.

Susan Fieldhouse: My first husband was quite outspoken and if he did see somebody that he did think looked better than me, he would always try to put my confidence down by saying so. Aw, look at that person over there. She looks better than you do or that dress looks better on her. Wouldn't look like that on you sort of thing. It's not like that with Stephen at all.

Female Speaker: Stephen is Susan's second husband. They've been together for 18 years and have four boys.

Susan Fieldhouse: If ever I say, oh I'm always a bit bothered by me sticking out tummy or anything like that, he always tells me I shouldn't be so down about myself. You know? He always says well everybody changes with age, love.

Female Speaker: Stephen is seven years younger than Susan and that's been playing on her mind.

Susan Fieldhouse: I kind of always wonder whether he might oh I don't know. It probably wouldn't be. It's probably just in my imagination but that maybe he might go off me.

Female Speaker: For the past few weeks, Susan and husband, Stephen, having been anxiously waiting to hear if she's been chosen to undergo a drastic transformation.

Susan Fieldhouse: God, I thought you were going to say I wasn't going then! Yes, I would! I would love to go. I really, really would. Thank you very much.

Female Speaker: Susan's life and looks are about to be changed forever. In Los Angeles , the Brand New You team is already planning radical intervention.

We've assembled a team of the world's leading cosmetic surgeons. They earn millions by transforming the looks of Hollywood 's rich and famous and they've made it their mission to create the woman Susan's always dreamt of being. Charged with rejuvenating her looks is one of the world's top facial cosmetic surgeons, Dr. Paul Nassif.

Paul Nassif: My initial impression of Susan is that she looks older than she is. Now the most prominent feature is her nose. Her nose is bulbous. It's pretty big and she has got a large hump.

Susan Fieldhouse: I would definitely want my nose to change. It's quite odd shaped at the top.

Paul Nassif: We can take care of all this. We can fix this for her.

Susan Fieldhouse: Everything is definitely sliding a lot often, ending up under here. I would like definitely to change that. Make it look more slimmer.

Paul Nassif: Everything comes down with gravity. So my goal is to do what I call an anti-gravity lift. And I want to lift everything up. I don't like the sideways pull.

Susan Fieldhouse: I would like my boobs not necessarily making them any bigger but maybe just a little bit more shape and put them back in because having four children does take its toll. And my tummy, I would definitely like that to be flatter.

Female Speaker: The guru transforming her boobs and tummy is leading edge cosmetic surgeon, Dr. Gerald Minniti .

Gerald Minniti: The first thing I notice when I look at a picture of Susan is that her belly hangs below her panties, her knickers. And that's ugly and I'm sure it's uncomfortable too. A tummy tuck is going to completely eliminate this problem. I also want to make her breasts bigger. She wants larger breasts.

Susan Fieldhouse: I kind of feel that if I have these things than I would feel better about myself and I'd probably project that a bit more.

Female Speaker: Sorting out her moles is pioneering skin specialist, Dr. David Amron .

David Amron: She's got these two nasty moles that I would just love to remove. So, I'm going to do that.

Female Speaker: Giving her a dazzling Hollywood smile, dentist to the stars , Dr. Bill Dorfman .

Bill Dorfman: Poor Susan, her mouth is a mess. Right away, I need to do three crowns down here. These teeth are just crumbling and then I'll do eight veneers on her upper teeth, six veneers on her lower teeth. She'll have a beautiful smile but this won't be easy.

Female Speaker: Coming up, Susan arrives in L.A. to begin her Hollywood nip/tuck. She goes under the knife in a series of extreme procedures to radically and permanently transform her looks. But it's not all plain sailing.

Female Speaker2: Why aren't things simple? You want jeans but now they are all boot cut, non-fit, twisted, low-rise, super loose! And white paint? No, you get moonlight, lily, New Hampshire with a bald white with a hint of pow tendril. As for younger, if it's not a whip, gelatin boost thing, it's a new fangled thing with sweetener and citric acid. Not for me. So I eat Ski . No artificial colors, sweeteners or preservatives. Just deliciously simple fruit yogurt. Ski fruit yogurt, keep it simple.

Male Speaker: From Nestle.

Male Speaker2: Morning, loser.

Female Speaker3: Everyone wants to be a loser. Scientific research shows a Special K challenge may help you lose body fat, not just water. Find out if you are a loser with your own Special K body fat analyzer.

Female Speaker4: Bill, this is Kirstie . I see your designer. She's a real loser.

Male Speaker3: Going my way, loser?

Female Speaker3: Send off for your own Special K body fat analyzer.

Female Speaker4: No Heat. No pics of the skinniest celeb yet and no latest on Jen getting over Brad. A world without Heat unthinkable.

Female Speaker5: Cherry lips maybe, perhaps soft pink or blushing candy but chocolate looks good. Purple flamer perfect. ______ lipstick, a stunning collection of lip color. Borjour.

Male Speaker4: Isn't this true why we became doctors?

Male Speaker5: No treating illnesses is why we became doctors. Treating patients is what makes most doctors miserable.

Female Speaker6: I just want to die with a little dignity.

Male Speaker5: A little dignity, you can't die with it.

Female Speaker7: House new drama, starts next Thursday at 10: 00 on 5.

Male Speaker6: Well good evening, Mrs. Fieldhouse.

Female Speaker: Forty-five year old, mother of four, Susan Fieldhouse has come to Los Angeles for a radical makeover and this is the very first time she has left England .

Susan Fieldhouse: I just want to look at everything and take everything in and try to remember everything to tell people when I get back home.

Female Speaker: She's here to have $300,000 worth of extreme plastic surgery.

Susan Fieldhouse: It's for me to feel better about myself, to have more confidence, to project a better image to people.

Female Speaker: It will be a life-changing journey. She'll need strength, courage and conviction to see her through because her transformation will be more restoration project than makeover, with hardly an inch of her body untouched.

It's Susan's first day in L.A. and she's going to meet her surgeons. They'll examine her and come up with a radical plan to transform her. Her first appointment is on L.A. 's equivalent of Harley Street , Spalding Drive . She's here to meet leading cosmetic surgeon, Dr. Paul Nassif. Remarkably, Susan is only 45 but she has all the classic signs of premature aging.

Paul Nassif: I'm looking at where the location of your eyebrow and I'm also looking at the crease of your upper eyelid to see how much extra skin you have on your upper eyelid right now. I'm just feeling to see how much of this is excess skin and how much of this is a little bit of fatty tissue in the neck. I'm also looking at the jowls of your face.

Susan's going to have a complete facial makeover. We're going to perform an endoscopic brow lift, an upper eyelid lift. We're going to do a lower facelift with cheek fat pad elevation. We're going to put tissue into her smile lines and we're also going to put Restylane into her upper and lower lips. And in addition to this, we're going to perform a rhinoplasty and also a septoplasty.

Female Speaker: It's major surgery and the most dramatic change will be her nose. Susan also wants to drastically alter her body. She's hoping leading surgeon, Dr. Gerald Minniti can transform her sagging breasts.

Gerald Minniti: In terms of symmetry, you've got good nipple position. The size is good. The only issue I have, Susan, is I feel like I wish that the nipples could be sitting a little higher. I think that implants alone would give your breasts a little bit of a lift but I think to achieve a look that we're going to find really attractive and rejuvenating, youthful is a breast where the nipple sits a little higher on the mound. And I think that would give you the best result.

With Susan I'm going to put her breast implants in through her belly button. I've been doing this for quite sometime and what I find is that women who have breast implants placed through their belly button have less pain. They recover faster and they just feel better.

Female Speaker: As well as a breast lift and augmentation, Dr. Minniti also plans to give Susan a flat, taut tummy.

Gerald Minniti: It's just this bottom bit. It's just from about here down that I would like to try and improve things. And that's probably where I'd make my upper incision. Somewhere right around here. And by doing that

Female Speaker: For the best result, Susan will also have extensive liposuction.

Gerald Minniti: And then regarding your belly would be liposuction of the entire abdomen from top to bottom and hip and flank area. And then followed by a mini-tuck. And the good news is all this can actually be performed in one surgery.

Female Speaker: Susan's final consultation is on the other side of Beverly Hills with America 's top dental surgeon, Dr. Bill Dorfman. He's the celebrity dentist and his client list reads like a who's who in Hollywood .

Bill Dorfman: Hi, Susan, how are you? I'm Dr. Dorfman.

Susan Fieldhouse: Pleased to meet you.

Bill Dorfman: When you smile, I'm looking at your front teeth and this one is like a mile longer than that one. I think it would look really cool if they were the same length. What's going on over here? Smile real wide for me. I think we're missing one. What happened there?

Susan Fieldhouse: They took some out when I was a child.

Bill Dorfman: They did like eeny, meeny, miney, mo?

Susan Fieldhouse: No because my mouth was too small to fit them all in.

Bill Dorfman: Now here's the problem. The tooth that you are missing over here that should be an eyetooth there. You don't have an eyetooth there. You have a bicuspid there. That looks kind of funky. We're going to convert this bicuspid into a canine. This bicuspid into a canine. I'll make this lateral and this lateral, both match and then we'll make this central and this central, both match. So you'll have symmetry from both sides and I think, look a lot prettier.

Female Speaker: Like her face and her body, Susan's mouth needs a complete overhaul. Her day is not over yet. Susan has one last stop to make. As well as her surgical transformation, Susan will have a total L.A. style makeover and hair guru, Chaz Dean has to begin that process now.

Chaz Dean: Due to the fact that Susan is going to be having facial surgery and the sutures will be in her scalp, we're going to go through and color her base color prior to, to avoid any infection.

Female Speaker: This is only a transition phase. Her full style makeover won't happen for a few weeks. Today has been pretty overwhelming. Tomorrow will be one of the biggest in her life. It's when her transformation process really begins.

Susan Fieldhouse: I'm excited about it but I'm kind of a little bit nervous as well because tomorrow is the biggest day for me for my surgery.

Female Speaker: The surgeons' plan to just transform Susan in just 28 days. Her first operation will be combined face and breast procedures followed by seven days recovery. Then liposuction and a tummy tuck, four days recovery. Then her nose will be reshaped. Next she'll have Lasix eye surgery. Her moles will be removed and finally she'll have zoom whitening and veneers.

It's D-day and time for Susan's first operation. To reduce the length of time she'll spend under the anesthetic, her face and breast stops will be carried out at the same time and so speed her recovery.

Susan Fieldhouse: Hello.

Paul Nassif: Susan, good morning.

Susan Fieldhouse: Hi. Good morning.

Paul Nassif: Well what I'm going to do right now is I'm going to make some marks on you. All right, I'm marking the little incisions here. Just kind of feeling out some of the muscles.

Female Speaker: Dr. Nassif's markings are the blueprints for his part of the operation.

Susan Fieldhouse: Wow.

Paul Nassif: It's a road map is what that is.

Susan Fieldhouse: It is, yes, like you could join all the dots up.

Paul Nassif: Exactly.

Female Speaker: Now Dr. Minniti can prepare his part of the operation, the breast enlargement.

Gerald Minniti: So these represent the little tunnels that I'm going to create to get up there.

Female Speaker: Susan's ready for the OR and Dr. Nassif scrubs in to begin the procedures.

Paul Nassif: I'm going to start from the top part of the forehead; do an endoscopic brow lift, followed by an upper eyelid lift. As soon as I'm finished with that, we're going to proceed with the lower face lift and then I'm going to take tissue from the face lift area and then place it in her smile lines.

Female Speaker: It's quite extraordinary to have two of the world's leading cosmetic surgeons simultaneously carrying out the one drastic transformation.

Paul Nassif: I'm going to elevate the eyebrow. At the same time, Dr. Minniti is going to start his breastwork and I'm elevating the brow right here. Go ahead, sir.

Gerald Minniti: I make a little horseshoe-shaped incision right in the belly button area. I actually take the fat right off the muscle of her belly and I continue right over the muscle along my little lines that I have drawn here all the way up to the breast. So what I've done now is I've created the pockets where the implants are going to be placed.

Paul Nassif: So what we are doing now is we are going to be using the endoscope. This is basically a small camera.

Female Speaker: The endoscope allows Dr. Nassif to operate with precision and means minimal scars for Susan.

Paul Nassif: I'm working on the muscles that pull the eyebrow down. As we snip this, the muscle, it helps weaken the frown line that she has.

Female Speaker: Meanwhile, Dr. Minniti is preparing to insert the breast implant.

Gerald Minniti: Roll the implant into a tubular structure and it should just roll right on up and it's now under the breast and under the muscle and my first implant is now in place and I'm going to fill it with the first 50 cc's of fluid. Both implants are in and one is filled. I'm filling the other.

Paul Nassif: That's big.

Gerald Minniti: It looks big now. We're going to fix the pocket in a second to make it appropriate for the implant. It's going to look a lot softer when I'm done with it.

Paul Nassif: So we're putting in two sutures on each side.

Female Speaker: Dr. Nassif's pioneering technique hides the scars in the hairline.

Gerald Minniti: Okay, what I'm going to do now is just sort of massage the implants in there sub-muscular pocket to make sure that there is as much room as can be because I'm going to now insert some devices, which are going to equalize the appearance of the pockets and make sure that they are symmetrical. Okay, great.

Female Speaker: The next stage is to reposition the nipple an inch higher on the mound.

Gerald Minniti: And I do that first with a stitch that is going to be permanently inside her breast and will not dissolve and it's going to bring this outer ring of skin towards the nipple.

Paul Nassif: While he's doing that I'm actually marking the upper eyelid to see how much skin I'm going to be removing. Okay. The stitch has to be placed perfectly. And if we do it right, which I'm sure I will, once she heals, she'll have a new imperceptible scar.

Gerald Minniti: What I see is very good symmetry. She's just a little bit bigger than I think she and I discussed. Although, probably she'd like that. Most women, if they have one regret after breast augmentation surgery is that they regret they didn't go a little bit bigger. All right, Dr. Nassif, I finished the breast surgery and I'm going to leave you alone.

I think in the beginning, especially, Susan is going to feel like she's got quite a large breast because there is swelling that exists right now. Fortunately it looks good right off the bat and only looks better with time.

Female Speaker: With the breast surgery complete, Dr. Nassif begins the lower face-lift.

Paul Nassif: We just made our incision through the skin. Now our next step is to what we call undermine or elevate the tissue, lift the skin up from the face. This is the key to the procedure - the stitches that lift up the muscle.

Female Speaker: The stitches tighten deep tissue to elevate Susan's cheeks, jowls and neck.

Paul Nassif: We can put a lot of tension on this area, whereas we don't want any tension on the skin closure because that's what will give you scars.

Female Speaker: Having tightened the muscles, Dr. Nassif can now remove the excess skin.

Paul Nassif: Okay, so I'm tailoring the earlobe, which is a very important part because if you do the earlobe wrong then you'll have that, again, weird pull on the earlobe and then people would be able to tell that she had a face-lift.

Female Speaker: The scar is concealed in the line of the ear. With a few final stitches, Susan's marathon operation is over.

Paul Nassif: Yeah, we're finished. We're just finishing up now.

Her face-lift did take quite a long time. But her brow lift, upper blepharoplasty, which is an eyelid lift, lower face-lift went perfect from my part. So now we just have to wait another week or so for doing her rhinoplasty. So from my standpoint, I'm happy.

Female Speaker: Susan can now return to her hotel, where she'll have round the clock nursing care. Twelve hours after her op, she's beginning to feel the strain.

Susan Fieldhouse: I think there is quite a lot of redness down here. I don't know. This side is really quite red and a little bit over that side but I think this is something I've got to concentrate on more. I've just got to keep lying down and putting ice packs on it he said and it will be okay then.

Female Speaker: For the next seven days, it's total rest and recuperation. This recovery period is vital if Susan is going to be fit enough for the next stage in her transformation.

Susan Fieldhouse: It's made a great deal of difference here because before you couldn't really distinguish my jaw line here. It kind of just it was all skin, you know and now I can actually see my jaw line, which is great.

Female Speaker: The recovery is remarkable. Tomorrow Susan will be ready for her next big op. It will be another joint operation with Dr. Minniti giving her a tummy tuck and Dr. Amron carrying out extensive liposuction.

David Amron: Well one of the things I noticed about Susan is that she needs some liposuction in her tummy both the upper and lower tummy and her hips and flanks. When people are fat in their stomachs, many times they're fat in the flanks, which is a very common area for liposuction.

Female Speaker: Dr. Amron's revolutionary technique of performing liposuction under local anesthetic means Susan will be awake throughout. And she finds this prospect daunting.

Susan Fieldhouse: Don't know if they'll be able to give something to calm down or not. I'm shaking like a leaf. I really am.

David Amron: I can understand that patients are little bit anxious about being awake during liposuction but there is so many great benefits this way of doing it and patients are not nauseous right after the surgery. They literally walk out half an hour later feeling great. And they take a walk the next day. They are very active right away. It's going to be a better procedure if we sedate you a little bit at least during the numbing. It's not that big of a deal. It is surgery but it's not as an invasive as people think.

Female Speaker: Dr. Amron's prescribed Valium to calm Susan's nerves. It's done the trick, within half an hour, her anxiety is gone.

David Amron: Reducing these bulges here it's really going to give a better shape to your butt.

Female Speaker: Heavily sedated, Susan is ready for her procedure.

David Amron: This fluid is not only numbing the tissue up but it's really helping to loosen it. So before I do liposuction it prepares the tissue well.

Female Speaker: Dr. Amron is targeting the pockets of fat where Susan is genetically disproportionate.

David Amron: She's got a good waist and her butt is in good shape but she's got this bulge on her hip. So you can see again, this nice, pure fat that is coming out. We're going to get almost two liters of fat out of Susan. We're talking about pure fat. This is not like a five-pound weight loss. Quite honestly, Susan would never be able to lose this much with diet and that's really what liposuction is all about.

Female Speaker: By the time Dr. Amron's finished with her flanks, arms, hips and thighs, he'll have sucked out almost ten pounds of fat and despite her fears, she seems to be coping.

Male Speaker7: Susan, you look very comfortable. Is that a correct assumption? Yeah, you are feeling a little bit of the pressure or movement as Dr. Amron's working?

Susan Fieldhouse: Just pressure.

Male Speaker7: Okay, good. Anything changes, please let me know.

Susan Fieldhouse: Okay, thank you.

David Amron: I can see and feel that it's completely smooth and even. There are no irregularities, no areas of excess fat remaining and that looks great. We're done. We're done. That's it.

Male Speaker7: What we're going to do is we're going to turn around to her back and then Dr. Minniti is going to come in and do the second part of the procedure, which is going to be a tummy tuck.

Female Speaker: Coming up, Susan gets the body of her dreams. Her despised hooknose is finally consigned to history and she experiences a modern day miracle.

Susan Fieldhouse: I can see!

Male Speaker8: It's the start of the British New Potato season and I've come to Jersey to see how Sansbury's go about sorting one of my favorite varieties, Jersey Royals . Wow.

Male Speaker9: You can still pick these early fields by hand.

Male Speaker8: Really. So what makes Jersey Royals so special then?

Male Speaker9: Well it's the climate and the richness of the soil. I'll lift you one.

Male Speaker8: Look at that. They look like little jewels don't they? You can tell they are fresh when they are stored loose and lovely like that.

Male Speaker9: Sansbury push to get these potatoes on the shelves within just a few days.

Male Speaker8: How do you like yours?

Male Speaker9: I love them roasted, fried but my favorite is boiled with lots of butter on top.

Male Speaker8: For me the best new potato in the world and they are in season right now. And they are at Sansbury's.

Male Speaker10: Walls Strawberry Cornetto .

Male Speaker11: Ha, ha. That was strawberry all the way down.

Female Speaker8: My mom says you are what you eat. So if you eat French fries, you'll become French?

Female Speaker9: Yep.

Female Speaker8: And if you eat Arilla beans, you'll become Arilla.

Female Speaker9: Yep.

Female Speaker8: And if you eat what's this then?

Female Speaker10: It's Green Giant Sweet Corn.

Female Speaker8/ 9: Green Giant?

Female Speaker10: Green Giant Sweet Corn is full of natural goodness and one of your recommended five a day fruit and vegetables.

Jingle: Ho, Ho, Green Giant.

Male Speaker20: Now there's new Oust portable fan.

Female Speaker13: It works. I love the Oust fan because you can tuck it up here or even with the smelly shoes.

Female Speaker14: It's great because it's not an overpowering smell.

Female Speaker15: You can put it anywhere.

Male Speaker20: The Oust portable fan continuously circulates its formula throughout the room to get rid of bad smells.

Female Speaker16: This whole room reeked of wet dog. The Oust fan gets rid of the odors.

Female Speaker17: For the first time, a survivor of fetal abduction tells her story.

Female Speaker18: We had to go beyond the idea that this is about having a baby.

Female Speaker17: And a woman accused tells hers. She stole my fetus. Monday at 9:30 on 5.

Female Speaker: After three weeks in L.A. , 45-year-old Susan Fieldhouse from Halifax has reached the midway stage in her total transformation. She's already undergone major face and body surgery but has another radical procedure a tummy tuck.

Gerald Minniti: Like so many women who have had children, sometimes the skin does not retain its elastic qualities and for Susan she also had a c-section and a hysterectomy. So she has a number of scars very low in her abdomen. However, the quality of her skin, up around her belly button and up above her belly button, is good. So, which makes her a perfect candidate for what we call a mini-abdominoplasty. I think we should easily be able to remove this much skin from her lower belly, which means this little tad of skin between her and the belly button is going to be pulled flat.

Female Speaker: Dr. Minniti starts the tummy tuck by making his incision from hipbone to hip bone.

Gerald Minniti: We have gone through into the fatty phase and I'm getting down to the muscle wall where I want to be. This is usually a little easier on somebody who has never had any surgery in this area but she's had a few surgeries so there are scar tissue right in the area where I want to be. So I have to be careful approaching the muscular wall.

Female Speaker: It looks brutal but what he is removing is redundant fat and tissue.

Gerald Minniti: This is the redundant, inelastic tissue.

Female Speaker: Dr. Minniti removes ten pounds of excess fat from Susan's body.

Gerald Minniti: It's not like losing ten pounds on a diet. When you lose ten pounds in a tummy tuck, you've lost an enormous amount of tissue in a very focal area.

Female Speaker: Having a tummy tuck will always leave a scar but Dr. Minniti makes this minimal by concealing it in Susan's bikini line.

Gerald Minniti: These are absorbable sutures.

Male Speaker7: That's going to look great.

Gerald Minniti: The surgery went perfectly. I like her new shape and I think she is going to heal nicely and I'm very excited that this is going to complete her total makeover.

Female Speaker: Remarkably only a few hours later, Susan is keenly inspecting Dr. Minniti's handiwork.

Susan Fieldhouse: I've had a little look at it and it looks flat. It looks nice. Dr. Minniti says it will just heal in a nice, just a faint line. He says I'll even be able to wear a bikini again. I didn't tell him I've never worn a bikini but never know, do you?

Female Speaker: Over the next few days, Susan's recovery speeds along. She's now ready for the day she's been dreaming of since she was a schoolgirl - reshaping the hooked nose she hates.

Susan Fieldhouse: He's going to take the bump out of my nose and he's also going to adjust some pieces of cartilage, which will actually help a problem that I have had for a while, which is a little bit of trouble breathing and I snore terribly and he said it will help that as well. So by this time tomorrow I should have a new nose. I'm really excited about that because that was a major thing that I wanted doing.

Female Speaker: Dr. Nassif plans to drastically transform Susan's nose in a four-hour operation.

Paul Nassif: So what we're going to do with your nose is what we talked about before make your tip smaller, take the bump down. I'm going to look at your nose because what I told you before, the way your cartilage is in your tip, it's going this way.

Susan Fieldhouse: Right, yeah.

Paul Nassif: And so we're going to make the tip smaller, your nostrils might collapse in more. So I might put a little piece of cartilage underneath here, you know, from your septum from inside your nose.

One of the difficulties with Susan's nose is that she has what we call nostrils that already are collapsing. We have to do something to keep those nostrils open. We have to pop them open with a piece of cartilage from the septum because if we didn't do that, prophylactic type of grafting to her nose, her breathing would obstruction would get a lot worse. Granted she would look a lot better but her breathing, her nostrils would collapse even more.

Female Speaker: Rhinoplasty is the most common but also the most complicated of all plastic surgery procedures.

Paul Nassif: I'm elevating the tissue off the septum right now. Okay, go ahead, do it again. We're actually removing the bony part of the septum that's deviated now. We're using this little this is called an osteotom and this hammer to help remove some of the bone here.

Female Speaker: This stage of the operation starts the process of clearing the nasal passage of obstructions so Susan can breath a lot easier.

Paul Nassif: Right now what we are doing is we are removing bony cartilage that is deviated into her airway. This is what's also causing a lot of problem with Susan's breathing on the left is this bone is pushed over into the left airway.

Female Speaker: He can now remove the bump that caused the crook on Susan's nose.

Paul Nassif: Now what we're going to do is we're going to start shaving off the hump of the nose. I'm cutting a piece of cartilage. This cartilage came from the inside of the nose called the septum. This is to widen the middle part of her nose. Middle part of her nose is just to darn thin. It's going to be a softer nose. It's not going to be a very cute, pretty nose because her cartilage is not going to completely allow us to do that. After three months, she'll look pretty darn good though but for the first few weeks, it's an acute change every week.

We're finished. Everything went great. Everything went absolutely fantastic.

Female Speaker: Despite the four-hour op, Susan is jubilant. She's waited a lifetime for this moment and even thought is pretty bandaged up, she can already see a dramatic difference.

Susan Fieldhouse: I can even sort of tell with the cast on that there is no bump there. And that, the tip of if there, it definitely does look a little bit smaller. So excited about my nose.

Female Speaker: Rebuilding Susan has been a monumental task. The invasive surgery is over but not the cosmetic procedures. A few days later, Susan has another appointment with Dr. Amron. As well as being a pioneer and liposuction expert, he's a groundbreaking clinical dermatologist.

David Amron: Susan has a couple of quite noticeable moles I would say. They are really easy to remove. They will take about a minute or two. I just numb them and shave them off. It's so quick. It actually takes longer to talk about it then to just remove it.

So I'm going to do what's called a shave biopsy on Susan's moles. I'm very, very gentle with injections so you are going to feel a tiny, little poke her. It's not brain surgery. It's not that difficult to do. On the other hand, it's got to be done really right otherwise you really can leave a person with an unattractive, unnecessary scar. Voila! That took about 30 seconds to do. The numbing actually took longer. Small, little bandage on that. Bingo!

Susan Fieldhouse: Thank you.

David Amron: Not too bad, huh?

Female Speaker: Susan has one more removal process that is just as important. Dr. Nassif is going to remove the cast and reveal her brand new nose.

Susan Fieldhouse: Hello.

Paul Nassif: Hello my little bruised one.

Female Speaker: Six days after surgery, she's ready to see the results.

Paul Nassif: Well for one week

Susan Fieldhouse: I can't feel that bump when you did that. I didn't feel the bump.

Paul Nassif: Take a look at this.

Susan Fieldhouse: Yeah, I can see even though it's swollen, I can see. If you would have done that before, I could feel where that bump was and I can't. That is so straight.

Paul Nassif: Look how straight this is.

Susan Fieldhouse: Thank you.

Paul Nassif: I told you.

Susan Fieldhouse: That is great. I'm so pleased.

Female Speaker: There's now less than a week remaining until her grand reveal but no let up in her schedule to transform her. Susan has already seen a dramatic improvement to her face and body but Dr. Dorfman has a lot to do to put the dazzle back in her smile.

Bill Dorfman: Smile really wide for me.

Susan's mouth is a mess. She needs three crowns right away on the lower. Then we'll do eight veneers on her upper, six veneers on the lower and with that, I can give her a really beautiful smile. Now the top edges of these teeth are all jagged so I'm just going to smooth those across here. This is real noisy.

Female Speaker: Susan's bicuspids are reshaped and molded to replace her missing canines. This will not only give her a better bite, it will give her a great Hollywood smile. Dr. Dorfman fits Susan with revolutionary Da Vinci veneers, a thin shell of porcelain that fits perfectly over the front of the tooth.

Bill Dorfman: With the veneer, I can change the shape, the color, the size and the position of your tooth all at once.

Susan Fieldhouse: Can you bite things

Bill Dorfman: Well if you bite things that are too hard, they will all fall off. I'm kidding.

Female Speaker: To build Susan's new smile takes eight hours and costs a cool $30,000.00. Once they are bonded, they will last a lifetime.

Bill Dorfman: You won't believe it. It's really beautiful. Ready?

Susan Fieldhouse: Yeah.

Bill Dorfman: One, two, three.

Susan Fieldhouse: Wow! Yeah, they are really great.

Bill Dorfman: That's the Susan we know and love.

Female Speaker: With the surgery over, it's time to transform her image with some Hollywood style vital if Susan is ever to recapture her femininity. She spends her life in casuals and hasn't been out of trousers in decades. Our fashion guru Kendrick Osorio has his work cut out for him if he is to transform her in time for her red carpet reveal.

Kendrick Osorio: My opinion of Susan is that she's lost her essence of being a woman.

Susan Fieldhouse: I don't want to accentuate the fact that I feel frumpy so I don't want to be wearing housewifey type of things. I do want to be a bit trendy.

Kendrick Osorio: She's very practical. She doesn't have any style and I'm going to help her focus and understand about reconnecting with being a woman.

Female Speaker: Susan also needs drastic intervention from our hair and makeup guru, Chaz Dean. He's already banished the gray. Now he needs to makeover her boyish cut into something more womanly.

Chaz Dean: Susan's hair is cut so short and the fact that she let it go grow, ages her completely. Not quite sure how much we can do with the cut but we definitely can do something with the color.

Female Speaker: Our gurus have just 48 hours to turn Susan around.

Kendrick Osorio: Fashion police.

Chaz Dean: Hello.

Female Speaker: To discover the full extent of their task, they are about to carry out their own unique style audit.

Kendrick Osorio: It's so dark in here. You know what I'm going to do? I'm going to take everything out and I want to see if I can understand where you were so I can know where you are going.

Chaz Dean: Hiding, no more hiding.

Kendrick Osorio: With the exception of the one shirt there, I feel like you are a boy. It looks like a boys closet. It looks like boys clothing.

Chaz Dean: She lives with -

Kendrick Osorio: This is a boys' pant.

Susan Fieldhouse: There's definitely

Kendrick Osorio: There's no sexuality there.

Susan Fieldhouse: Cover up.

Kendrick Osorio: I'm not in love with that. So, if it's okay I'm going to toss this over the balcony. I'm joking!

Female Speaker: As far as our gurus are concerned, Susan needs to swap masculinity for femininity and show off those fabulous new curves.

Kendrick Osorio: I'm not going to go with the tits and ass, all the really bright colors. I'm going to go with the very sophisticated, sultry, just touching and embracing her silhouette. The issue with Susan is this. She has not allowed herself to embrace her femininity. Now what you do in this case, is you shock her and you give her nothing but feminine clothing and she will not dare go back to the old way of living.

I have a very good friend of mine by the name of Richard Bowman and his forte is femininity. The bling, bling, the tassels, the embellishments. This is the perfect person because he understands about making a woman feminine.

Female Speaker: In 48 hours the brand new Susan will be revealed on the biggest night of her life. She won't know what killer frock Kendrick has chosen until she's ready for the red carpet. She'll have the frocks and the rocks but can anything be done with her cropped hair?

Chaz Dean: She's modeled her look after a man's, which isn't a good idea if you are a woman. So either she's just let her appearance go or she's found it really difficult to find a great hair stylist.

Susan Fieldhouse: It's going to be really interesting because he was hoping it would grow a bit and it has grown a little bit but not sure what he's going to do with it.

Chaz Dean: We're going to go through and add layers to soften up her look and I'm going to go through with really rich caramel and honey tones to warm up her overall look.

Susan Fieldhouse: And he's good at what he does so I'm sure he'll be able to manage something with my hair.

Female Speaker: This is merely the preparation. Chaz will add the finishing touches when it's time for her red carpet reveal. And that's just 36 hours away.

Over the past six weeks, Susan's external appearance has changed dramatically. But there is still a bit of her old self remaining.

Susan Fieldhouse: God it was terrible when I was at school. Yeah, it was all the specky, four-eyes. That's what everybody used to call me. But then again they called everybody who wore glasses that. I hated wearing them. I absolutely hated it.

Female Speaker: Today it is time to say goodbye to her glasses forever. Susan hasn't ever known perfect vision. She's worn thick glasses from the age of three. The specialist who will give her her sight is Dr. Boxer Watler . He's one of the best refractive surgeons in the world.

Boxer Watler: How are you feeling?

Susan Fieldhouse: Okay.

Boxer Watler: Okay? That sounds like you are a little nervous?

Susan Fieldhouse: I am. It's kind of a big thing for me this, to be able to see without my glasses.

Boxer Watler: Been thinking about it for a long time?

Susan Fieldhouse: I have, yeah. It's exciting.

Boxer Watler: It is exciting.

Female Speaker: Dr. Boxer Watler takes an image of Susan's eye that's as unique as a fingerprint.

Boxer Watler: Captures over 200 points of vision distortions in your eye and we're going to put that on a computer disk and put it in the laser. Perfect.

Female Speaker: This truly is miracle science. A lifetime without vision can be corrected in minutes.

Boxer Watler: So this is actually where her distortions are. This is a unique little pattern here. And so this map shows red is where it's a little bit of a different level than the green. So this is going to be downloaded on a computer disk and we're going to put that information in the laser and the laser is going to use this to smooth it out.

Perfect. You want to concentrate on that orange just like you are doing. Keep concentrating right on the center of the orange there.

Female Speaker: It's virtually painless and the results are permanent.

Boxer Watler: Lift your chin just slightly.

Female Speaker: The laser erases all imperfections and in minutes it's over.

Boxer Watler: Perfect.

Female Speaker: Susan has the miracle of being able to see unimpaired for the first time in her life.

Susan Fieldhouse: I can see clearer.

Boxer Watler: Can you see the time up there?

Susan Fieldhouse: Yeah. Wow! That's great. Thank you. Thank you.

Boxer Watler: And it's okay to just dab here but don't rub your eyes. It's a big moment for you, isn't it?

Susan Fieldhouse: It is. I can see.

I could not believe it would happen so quickly and that made me cry. I sat up and I could see the clock. Before I couldn't see the wall never mind the clock.

It's fantastic. Thank you. I'm so pleased.

Boxer Watler: That's part of the brand new you.

Susan Fieldhouse: Yeah.

Female Speaker: Coming up, the big reveal night, the culmination of all her hopes and dreams. After $300,000.00 worth of cosmetic surgery and dentistry, the brand new Susan Fieldhouse is revealed before the gurus who transformed her.

Female Speaker: For 46-year-old Susan Fieldhouse from Halifax , the waiting and the hard work is almost over. Tonight is her Hollywood red carpet reveal.

Susan Fieldhouse: Oh, I feel lucky to have been selected. I'm glad it was me.

Female Speaker: With her Cinderella moment only hours away, a team of beauticians are pulling out all the stops to make her look the best she has ever looked in her life. The woman she is now couldn't be more dramatically different from the desperate housewife who left Yorkshire six weeks ago.

Susan Fieldhouse: I feel dumpy, frumpy, not really got all that much shape.

Female Speaker: To make her a brand new you, the world's leading cosmetic surgeons stopped at nothing to create the woman Susan always dreamt she could be.

Susan Fieldhouse: You can see little changes everyday. It's really good.

Female Speaker: She hasn't worn in a dress in two decades and her face hasn't seen a lick of makeup for years. But all that's changing. Guru Chaz Dean is adding the final touches of Hollywood glitz and glamour.

Chaz Dean: For Susan's reveal party, I really want to go in and play up her eyes. She's had the Lasix surgery. She's got gorgeous blue eyes. So I want to play them up soft, subtle, sexy and then her lips tend to be very tight and very clenched, so I want to do a very soft, light lip color to plump them up and enhance them and be really soft and feminine.

Female Speaker: Susan will remember tonight for the rest of her life. She's whisked away to the Hollywood Hills for the final touches to her amazing transformation.

Susan hasn't seen herself in a mirror so has no idea how she looks.

Susan Fieldhouse: Hopefully it still does look like me but a new, improved version.

Female Speaker: A few final touches and she's ready to flaunt her brand new look to the gurus who transformed her.

David Amron: I think the biggest change when I look at Susan is what a woman! I mean she looks amazing. Actually shocked me. It really did. I mean her hair looked great. Her face looked great. My work specifically, I really was happy with things. I think that it really brought her waistline in and just gave her a really nice shape.

Kendrick Osorio: Take a deep breath. How do you feel my love? Ready for this? Okay. What do you see?

Susan Fieldhouse: That's not me in the mirror is it?

Chaz Dean: That's you. That was always you.

Kendrick Osorio: Look at those eyes. Look at those eyes.

Chaz Dean: You were always being beautiful.

Kendrick Osorio: What are your boys going to say when they see you?

Susan Fieldhouse: They'll be speechless. They've never seen me like this before.

Female Speaker: What a transformation. The team have successfully turned the clock back. They rejuvenated her face and drastically changed her body. Now she looks fabulous and feminine.

Kendrick Osorio: Now you are going to go home and hold on to this, right?

Susan Fieldhouse: I promise yes.

Kendrick Osorio: You promise?

Susan Fieldhouse: Well I didn't come all this way for this shape to waste it when I get home.

Female Speaker: To create the brand new Susan, took six weeks, the world's finest cosmetic consultants and cost almost $300,000.00.

Susan Fieldhouse: It's for me. It was definitely beat for me. My nose, I've wanted doing since I was 14. It was really great. This has been a really good, good thing for me.

Paul Nassif: She looks fantastic. Her husband is going to just go crazy when he sees you.

Susan Fieldhouse: I feel like a girl in these. I mean a dress on definitely makes you feel different and now that your aware it's really, really nice. I definitely feel feminine.

Female Speaker: Susan is delighted with the transformation but what will family think of her $300,000.00 makeover?

Back home in Halifax, husband Steve and the rest of the family are anxiously waiting to see her.

Steve: I'm just waiting to see. I can't wait to see actually. It's going to be a big surprise for sure.

Female Speaker30: I just can't put together a picture in my head of what she's going to look like. I can't even begin to imagine because she's had so much done so it's going to be a complete surprise when she walks through the door.

Susan Fieldhouse: Can I come in?

Missed you.

Female Speaker30: Oh my God. You look fantastic.

Female Speaker31: Grandma, why do you have your glasses off?

Susan Fieldhouse: I don't need them. I don't need to wear them anymore now darling.

Female Speaker32: The warts on the face, no glasses, the hair is different. The boobs are different.

Steve: I wasn't going to say that but yeah.

Female Speaker33: Gosh, she looks wonderful.

Female Speaker34: She looks absolutely amazing.

Female Speaker33: Gorgeous.

Female Speaker: It's three months since Susan returned home. Plastic surgery has altered her looks but how much has it changed her image of herself?

Susan Fieldhouse: When I look in the mirror, I see somebody who doesn't look frumpy anymore. I look younger. I look a better shape. I didn't feel particularly girly before and I think if you look the shape of a girl, you can't help but act more like a girl.
