Lucy: Today you're gonna learn, just, how to draw some things that would help you in school. Today we're gonna start with drawing flags. You start over here, and it has to be pretty wiggly like that.
My name is Lucy Cove, I'm 41 years old, and I'm gonna have the rhinoplasty with Dr. Nassif
I got this bump from when I was 9 years old, I was riding my bike and I crashed on the sidewalk. The handlebars hit my nose. The scab went away, but I could tell that my nose wasn't the same. I remember falling off my bike and being all bloody, and my leg was all bloody, and yeah, going back to school feeling different.
Female Speaker: I do remember you being really embarrassed after that, waiting for it to go down, and it didn't go down.
Lucy: I'm recently divorced and I'm raising two kids, Kimberly and Brian. I think once I got married, I had a little bit less of a self esteem. I guess I was pregnant with Kimberly, and he goes, "You know, your nose, that bump on your nose, it's not hereditary is it?"
Female Speaker: Oh really?
Lucy: I thought that was mean.
Female Speaker: Yeah.
Kimberly: My name is Kimberly and I'm 16. The divorce was a big change, and sometimes change can be scary. It's so hard on me and my brother, just kind of hard for me to like talk about right now.
Brian: My name is Brian, and I'm 11, and I'm Lucy's son. My mom, she doesn't like how her nose looks. She really wants to get it fixed.
Female Speaker: Well Lucy, the girls have been working hard on a special cheer for you to give you some encouragement and inspiration.

Michael: My name's Michael and I'm 47 years old, and Lucy and I are boyfriend and girlfriend for about the past four months. Lucy's decision to get the surgery is, you know, it's her decision. I told Lucy that I would be there for her.
Lucy: Michael didn't ever say like, "Oh my god you need to get your nose fixed" or anything like that, but once I told him I wanted to do it, he was right there for me and told me he would be with me all the way, and I feel very supported.
Kimberly: I don't think my mom would go through the surgery if Michael wasn't in her life. He's such a kind person, and he treats my mom so well.
Lucy: Now we're really gonna look alike.
Kimberly: Yeah.
Dr. Paul Nassif: Hi.
Lucy: Hi.
Dr. Paul Nassif: How you doing Lucy?
Lucy: Good, thank you.
Dr. Paul Nassif: Nice to see you again.
Today I'm seeing Lucy Cope. She's coming in for pre-op today for her septorhinoplasty, and remove the bump from her nose.
You can't even see inside of her nose. You see that? We'll get that taken care of. Now that might have been from your trauma you had at age 9.
Lucy: It was a pretty bad fall.
Dr. Paul Nassif: I perform rhinoplasties all the time. But for a patient such as Lucy, I mean, this is a major thing in her life. Lucy is very unhappy with her nose. She also recently got divorced. Now is the right time for surgery. She's got the right motivation, and she's got realistic expectations. And once we take care of her nose, this will just help her move on in her new direction.
This is something that I'm pretty comfortable with, that's our goal. Something like this.
Lucy: Yeah like that, straighter.
Dr. Paul Nassif: Less bulbous.
Lucy: I feel really good about the consultation, I have a lot of confidence in Dr. Nassif. I think he's gonna do a great job.
Dr. Paul Nassif: Good, glad you like that.

Tonight we're expecting the organizers from our Face-to-Face Foundation, and caterers. They're gonna come help get us ready for our fundraiser. This is my wife Adrian.
Adrianne: Hi, I'm Adrianne, hi.
Rhonna: Hi Adrian, I'm Rhonna.
Dr. Paul Nassif: The Foundation helps victims of domestic abuse. When they look in the mirror, they see scars. They'll see a fractured nose or a big scar across their face. We wanna eras those scares as best we can.
Adrianne, I mean, I know you love carrying that dog, but
Adrianne: Can I say, well at least it's outside?
Dr. Paul Nassif: Tell me that's not like that.
Adrianne: My dog, he's very precious dog, but he-
Dr. Paul Nassif: We'll be back in one second, I'm sorry. Jeez.
Adrianne: My husband has a love-hate relationship, I better put him away.
Dr. Paul Nassif: Why can't we flush this down the toilet himself?
Male Speaker: Beautiful backyard.
Rhonna : It is. It's gonna be a great location for a party.
Dr. Paul Nassif: At our fundraiser, we're gonna have a poker tournament, a tarot card reader, and all kinds of entertainment. It has to be a wonderful, memorable evening, anything less than that just won't work.
Adrianne: As far as I know there's gonna be three shuttle busses, and a number of limousines bringing the guests up. My husband thinks they're big greyhound busses, but they're shuttle busses.
Dr. Paul Nassif: Hold on, hold on. there's no way that these busses are gonna make it in our small driveway and get out.
Adrianne: That's a big driveway.
Dr. Paul Nassif: No, they can't get out.
Adrianne: Oh my god, so you're telling me you can't, you have construction in here all the time.
Adrianne: Paul is clueless. He has no idea
Dr. Paul Nassif: Why don't we make a little bet on this?
Adrianne: Uh, let's move on.
He's gonna be wrong, that's the problem, cause he's always wrong.

Lucy: Well that's what I think I look like.
Dr. Paul Nassif: Well, I think you've accentuated it a bit.
Lucy: A little. And this is what I think my nose is going to look like.
Dr. Paul Nassif: Okay.
Lucy: I'm a little big nervous about doing the procedure. So what I try and think about it s the support that I have. Michael is very supportive of like anything that I do. That has really encouraged me to actually go through with it, so I feel really good about it.
Dr. Paul Nassif: Oh wow, this is the first mark-up I've seen.
Lucy: This is like, kind of something I've been really wanting to do, and then I have so many people supporting me.
Michael: She's at a point in her life when it's good for her to do it. She feels like she has the support. Life is taking on a new direction, and she's excited about it.
Lucy: I just wanna get it done, and have it kind of behind me.
Michael: Well, your nose will always be in front of you, but
(Laughter)
Michael: Sorry.
(Laughter)
Lucy: It won't be behind me.
Michael: Unless they really mess up.
(Laughter)

Dr. Paul Nassif: Today we're going to our surgery center, and we're doing Lucy Cove's nose surgery.
Nice IV, look at that. Yeah.
Lucy: It's surreal. I can't even believe I'm doing it, and can't even believe that it's really happening. You know, it's not a surgery that it's an emergency and I have to do it. So the anxiety part, it kind of wore off.
Is that my happy medicine?
Anesthesiologist: That's the happy medicine.
Lucy: Woohoo, happy!
Anesthesiologist : Just close your eyes and go to sleep, alright?
Dr. Paul Nassif: Okay, we're all set.
Some patients will have a very slight irregularity on their nose. To everyone else, they won't even notice it. But for that person, it bothers them. In Lucy's situation, I mean, she's been unhappy with her nose for a long time. And with one surgery, we'll make all those worries go away.
Let's take some cartilage that's deviated.
When I perform a septoplasty, I am fixing a deviated septum so that the air will go in the nose without any nasal obstruction.
We're gonna start moving part of the deviated bone; we'll use this cartilage for grafting inside the nose. And whatever I don't use, I'm gonna put right there.
Cartilage is tissue that's found in the nose. I can take it out, change its shape, and then put it back in. It will stay there.
A rhinoplasty is when I remove the hump on her nose, and also define her nasal tip.
So what we're doing now is removing a portion of the cartilage that is making her tip bulbous. Okay so what I'm doing now is I'm going into part three, which is removing the hump.
Alright so she has some asymmetry of the tip, we gotta put these other pieces of cartilage in. So we have to make little pockets for the cartilage slip in here. Sometimes it fits in easy, sometimes it doesn't. Alright so we're gonna finish closing the tip of the nose, and then we're done. She said she's waited many, many years. The whole goal is I wanna make sure that after waiting this long, she's really happy with it. So this is about as symmetric as I can make it. Okay so I think we're done here.
Lucy: I feel so relaxed.
Nurse: You feel relaxed?
Lucy: Yeah.
Nurse: Are you warm enough, Lucy?
Lucy: Yeah.
Nurse: Okay hon.
Dr. Paul Nassif: Everything went great. It was a little bit more trickier than I thought. Right when I was just about to finish, I took a look down, I noticed that there was some asymmetries present. Took some extra time to fix it, but when I was done, I was extremely happy. I mean, her nose looks great.
Everything went fantastic.
Lucy: Yeah?
Dr. Paul Nassif: Fantastic. You're gonna love your nose. Wonderful. You're a little groggy right now, so I'm gonna let you rest.
Her old nose was affecting her self esteem. In this situation, at least the way it looked on the table, and if she heals great, she'll be absolutely wonderfully happy, and her self esteem issue, hopefully will be absolutely gone.
Michael: Yeah, someone came to visit you here.
I can already tell you that your nose is less bulbous. I just wanted to use the word bulbous in a sentence, because I like it. It's a funny word.
Lucy: I'm tired. But I feel happy too. You just can't tell on the outside, okay?
Dr. Paul Nassif: Alright, she just paged me, now can she answer the phone please?
Adrianne: Hello?
Dr. Paul Nassif: Adrian?
Adrianne: Yes.
Dr. Paul Nassif: Hold on, something's leaking out of this refrigerator. What the heck is this?
Adrianne: Paul.
Dr. Paul Nassif: Yeah Adrian?
Adrianne: You know, I've got the caterers here, and they wanna know how many people are attending.
Dr. Paul Nassif: I don't know exactly how many people are coming.
Adrianne: Well they're asking me, and personally it's not my event.
Dr. Paul Nassif: Something's leaking out of this thing. Diana?
Adrianne: Paul, focus.
Dr. Paul Nassif: Use your best I mean, you're very good at this. Use your best judgment.
Adrianne: Well do you know who's showing up?
Dr. Paul Nassif: Well yeah, a bunch of doctors.
Adrianne: Paul is leaving me alone to deal with this fundraiser. I'm not very happy about that.
What time do you think you're gonna be home?
Dr. Paul Nassif: Good question.
Adrianne: Alright honey, I know.
Dr. Paul Nassif: I don't know how things are going. I've been so busy in surgery, I'm sure Adrian's doing a great job putting it all together.
Adrianne: Call me when you get done. [hangs up]

Lucy: Hi.
Dr. Paul Nassif: Oh, both of you are here. How you doing? How are you healing? You're chewing gum.
Lucy: Yeah.
Dr. Paul Nassif: Oh.
Lucy: Is that bad?
Dr. Paul Nassif: We don't wanna do that for a while. Just because I don't want your nose moving that much.
Lucy: Oh. It gives me better breath.
Dr. Paul Nassif: You might have to gargle then. Take that out for me.
Today is Lucy's one week post-op check.
Lucy: Oh yeah, it's straight. I like that. I don't look like Van Gough anymore!
Michael: It's very straight.
Dr. Paul Nassif: The hump is gone, it's nice and symmetric.
Lucy's nose, it's nice and symmetric, which is the most important thing for me. The only thing is, she's a little bit swollen, swollen all the way. So when you look at the front, you'll notice you're swollen in the sides and the bridge of your nose. You haven't been doing any running activity have you?
Lucy: No, just cleaning my house.
Dr. Paul Nassif: A little bit of activity can call a lot of swelling in the nasal region. If she were to take it easy, she probably wouldn't have been as swollen as she is. But usually when someone comes in with that much swelling in the tip of the nose, it means they haven't been taking it easy.
Lucy: I know cause my
Dr. Paul Nassif: You're supposed to take it easy as I've told you in your instructions.
Lucy: I did.
Dr. Paul Nassif: Well not housecleaning. I mean, you gotta kick back for one complete week.
Lucy: You're right. My house just really bugs me when it's a mess.
Dr. Paul Nassif: Lucy didn't follow all of our instructions properly. It's very frequent that I have to remind patients that they had major surgery. Usually you don't have a lot of pain with rhinoplasty, you feel pretty good after the second day, so they do a lot of activity. And that's not great for healing. They have to relax, they just had surgery.
So you need now to take it easy this week.
Lucy: I'm gonna take it easy for a week, take really good care of myself so I don't have any complications or much longer healing process.
Michael: Good job.
Dr. Paul Nassif: My pleasure.
Lucy: I really like it, thank you doctor.

Lucy: You know I have to kind of take it easy.
My recovery has taken a little longer than I expected, because there's been a little more swelling. It was just uncomfortable, but not painful.
Michael: The nose that she was living with for the last 30 years or so was not her nose. It was a nose that was disfigured by a childhood accident At this point in her life, she was able to fix what was broken.
Kimberly: I really feel like my mom has been transformed by this surgery. I've always known my mom was beautiful, like on the inside and out, but I think her confidence has just skyrocketed.
Lucy: I feel like I'm so much more comfortable looking in the mirror. I could just look at myself and just go, "Wow it's so smooth", and just not having to worry about that.
Michael: I think your nose looks almost identical to Bryan's, that's
Lucy: Cause your nose goes up a little bit it's very straight and it goes up.
I don't have any regrets on going through with the surgery. I'm really happy that I did. The only regret I have is that I didn't do it any sooner. It's not a distraction in my life now, I can focus on other more important things.
Kimberly: So I guess it does change your personally for the good, huh?
Lucy: I think it can, yeah.
Michael: If you're still under the influence of medication.
(Laughter)

Dr. Paul Nassif: First of all, I'm driving home. I have to go through these crazy streets of LA.

Dr. Paul Nassif: That's the number we would like to have. You're doing pretty well.
Adrianne: According to the tarot card reader, we're supposed to have three children, so I hope she's right.
Tarot Reader: I think you're gonna be pregnant in like a year. If not by the end of the year.
Adrianne: Really?
Tarot Reader: I mean, soon.
Dr. Paul Nassif: This is a charity event. To get a seat at the poker table, you have to donate $2,500 to Face-to-Face.

The fundraiser went great. I mean, this is for a great cause, it's for Face-to-Face. It really helps people. We raised a good amount of money. It's fantastic.
