Comment: In the oblique pre-operative view (left), the patient has a moderate amount of lower eyelid fat and a "tear-trough" deformity (hollow-appearing ridge near the corner of her eye). Standard fat removal would make the hollowness worse. In fat repositioning, the fat is repositioned into the hollow region making the "tear-trough" either less hollow or not deeper.
