PREOPERATIVE DIAGNOSIS:

  1. Nasal obstruction caused by deviated septum and hypertrophic turbinates. External Nasal Deformity.
  2. Bilateral compensatory hypertrophic turbinates.
  3. Septal deviation; left caudal deviation and left bony spur.

PROCEDURE:

  1. Complex septal reconstruction, submucous resection with major partial vomerectomy for correction of deviated nasal septum.
  2. Submucous resection and partial resection of right inferior turbinate.
  3. Submucous resection and partial resection of left inferior turbinate.
  4. Open rhinoplasty, platelet gel application.

FINDINGS: Left caudal deviation, left posterior bony deviation and spur, asymmetric tip with right lower lateral cartilage (LLC) more convex; ptotic tip,tip over-rotated and over-projected, thick skin.

COMMENT: This patient desired better breathing and to improve the appearance of her nose. 4-month post surgery photos reveal an improved profile. Patient continues to heal.
